HHS offering online education & training materials on Multiple Chronic Conditions (MCC)

Now available online: Health and Human Services created "a six-module curriculum designed for academic faculty, educators and trainers to inform healthcare professionals caring for persons living with multiple chronic conditions. Based on the MCC Education and Training Framework (http://www.hhs.gov/ash/initiatives/mcc/education-and-training/index.html), these modules provide knowledge and tools healthcare professionals can use as they improve quality of care of persons living with MCC.”

An introductory PowerPoint and six presentations are now available for use by educators across the healthcare curriculums teaching healthcare professionals about multiple chronic conditions (MCC). Topics include:

  • person and family centered care, 
  • self-management, 
  • interprofessional collaboration, 
  • knowledge for practice in complex patient care,
  • coordination of care delivery, and
  • systems-based practice

All are available in PDF or PPT format. 

HHS states that "The presentation can be saved, modified, and used in your presentations with healthcare professionals at any state of education (undergraduate, graduate, or continuing education." Click here for all the modules.
